Reporting bus incidents

This page provides information about the reporting of incidents to PTSV by accredited bus operators. If you are an accredited bus operator, this information is important.

Immediate reporting

If you are an accredited bus operator, a condition of accreditation is a duty to notify PTSV (through the duty officer) immediately after becoming aware of a ‘notifiable occurrence’.

For reporting purposes, a bus notifiable occurrence is:

  • Any incident that results in the death of a person
  • Any incident that results in any person being seriously injured (a serious injury is one that requires immediate treatment at a hospital or other medical establishment)
  • A collision that results in significant damage to any property or equipment (more than $15,000)
  • An occurrence where a driver in charge of a bus operated by you (as the accredited operator) tests positive for exceeding prescribed quantities of drugs or alcohol whether in connection with an accident or not, if known, and
  • an occurrence that may result in general adverse publicity for the bus industry.

Written reports

Once a bus operator has notified the PTSV duty officer, they must follow up with a written report of the incident within 72 hours of becoming aware of the incident, using the approved bus notifiable occurrence form. Contact us to obtain the form.

Who should provide the report?

All accredited bus operators have a duty to report to PTSV. However, individual bus organisations may have their own individual policies and procedures in place to ensure they meet their duty for reporting.

For example, some organisations may specify a particular position as being responsible for reporting, others may encourage staff in the course of their work who are ‘involved’ in an incident to notify PTSV.
